The State of Illinois requires all kindergarten students and any child entering an Illinois school for the first time to have a vision exam per state law by October 15th.
We have attached our vision flyer in English and Spanish, which includes locations CPS has partnered with to provide vision exams and glasses (if needed) for CPS students at NO COST. Parents may select any of the locations listed or their own healthcare provider to complete the state-required form.
